Welcome to the 2024-25 Moorpark High School Sideline Cheer Tryout portal.  We are so excited that you've decided to take on this incredible and exciting next step in your journey to be a MHS CheerLEADER.

Please follow the steps below to complete requirements for tryouts:

Steps 1-3 Must be completed no later than May 5th at 3:30 pm

STEP #1 TRYOUT APPLICATION

• Complete the Tryout Application by submitting the Google Form below. 

  • To complete this form, you will need to take a photo of yourself with your hair in a ponytail wearing a black t-shirt or tank to submit. No extreme makeup. You should look like you will look at tryouts.

  • You will need to provide email addresses for 3 teachers to complete a recommendation form for you.

  • You will need a photo of your grades from the last reporting period. Your grades can be a screenshot from Q. Photo must show your name and all classes.

  • You will also need to complete your Athletic Clearance as part of the Application, see step #2 below.  ​​

STEP #2 ATHLETIC CLEARANCE AND SPORTS PHYSICAL

• Complete the athletic clearance application by following the directions on the MHS  Athletic Clearance website.

• The sport you are applying for is, "Sideline Cheer" in the application. 

• There is a  tutorial on the login page but if you need further instructions, go to the following link:  ATHLETIC CLEARANCE INSTRUCTIONS

• Email Coach Tedrick at ctedrick@mrpk.org your sports physical form signed by your physician.  Physical form is located here:  MHS Athletic Emergency Medical Info Participation Form

• If you are unable to complete your physical in time, please notify Coach Tedrick via email.

• You will need to turn in the original signed copy of the Informed Consent form to Coach Tedrick by  Friday, May 5  with all other forms, see below in step #3.

STEP #3 FORMS AND CONTRACTS​

• Complete All forms given at the Parent's Meeting and return to Coach T no Later than 3:30 on May 5th. Forms can be brought to classroom D-7 or left in Coach's mailbox in the front office at MHS.
